Data Transformation

The CentriHealth IHR Information Model holds and organizes information according to its significance to each individual and their care teams. This transformed data is the basis for presentation of information to each stakeholder group and the active management of care across providers and over time. This unique model delivers:

• A comprehensive health information schema that sorts through the fragmentation of multiple transactions related to the same event (e.g., prescription, refill, claim, etc.) and creates a single, clean and optimized record of health on an individual;
• A clear "understanding" of the data.  The purpose-built IHR health ontology provides the ability to organize and filter information around concepts of clinical importance such as a disease state or a clinical specialty, rather than around the kind of test or the department in which a procedure was performed;
• A clear record of the individual's healthcare events across providers and over time. IHR receives data in many fragments from many systems. It analyzes this data to determine if they need to be incorporated into an existing healthcare event or to create a new one; and,
• A source of transformed information to third party systems in formats and using methods native to such systems, irrespective of the formats in which data was received.
