Supporting Tele-Services (Telehealth, Telemonitoring and Telemedicine)

entriHealth’s Individual Health Record (IHR) provides a new approach to supporting Tele-Services in a manner that focuses on addressing the challenges of integrating individual Tele-Services into the wider collaborative care environment. The IHR supports Tele-Services by delivering personalized care coaching, relevant content, products, and disease management solutions that have been proven to enhance outcomes and reduce costs. The IHR uses appropriate language along with visual simplicity to effectively engage individuals in understanding and participating in their health and care. The IHR enables caregivers and consumers to interact using the same record in different ways. Health care executives are often daunted by too many systems delivering too little integrated value. What is required is a supporting infrastructure that accommodates the detail of the Tele-Service, integrates with existing clinical care systems and engages with the individual in an appropriate and effective manner.

New Models of Care

Private and publicly funded healthcare organizations are contending with the economic and social implications that burden people that live with long term chronic health conditions. 'Tele-Services' are often becoming the solution of choice to address this issue ranging from isolated deployments such as Smartphone SMS/Text services right through to community-wide long-term care programs. All too often these initiatives are undertaken in isolation and seemed at first to offer self-evident benefits to both individuals and those providing them with support. But realizing those benefits of improved quality and reduced costs has proved more challenging than was thought.

New models of care are being developed that embrace both health and social care while supporting the collaborative care environment required to deliver quality care to people. The challenge is to support these newer models of care, often home-based and away from the familiar institutional boundaries of hospitals, clinics, and practices. If services to the individual end up piecemeal and lacking overall coordination then the result is fragmentation, duplication, and hence reduced effectiveness and increased cost.

A key economic goal is to manage the individual in the most cost-effective setting – large savings can be made by preventing use of and reducing reliance on high-cost care resources such as acute hospital admissions. Key goals for individuals themselves are to stay as well as possible, to be confident that they are doing the right things, and to stay out of the hospital.

IHR Models of Deployment

CentriHealth’s IHR is being used as a key component in Home Based Support, Advice and Monitoring services. These services are targeted at the monitoring of Long Term Conditions including diabetes, asthma, heart disease, hypertension and COPD. These services deploy a combination of Personal Monitoring Services of a person’s status including: Blood Glucose; Blood Pressure; Weight Management; Blood Oxygen; Peak Flow; Health Status and Exercise. These services provide one-to-one personalized advice & guidance from dedicated specialized nursing staff along with access to Self-Management tools for individuals via a web-based personalized Individual Health Record portal.

Telemonitoring of Home Hubs, Telecare Devices, Dispersed Alarms, Telehealth Devices and Self-Care Condition Centres are provided using low-cost, manufacturer independent, devices and services. Technical and Clinical Triage is provided by dedicated control centre agents using the integrated CentriHealth’s IHR. The IHR is a unique knowledge base environment that brings together relevant aspects of a person’s health status along with clinical guidelines and specialized health knowledge.

Advice is provided directly to the people who are showing early signs of exacerbation via one-to-one nursing advisors and a personalized IHR. The IHR automatically raises alerts based on clinical guidelines. Most interventions are handled entirely within the service with only the most urgent cases referred back into the care system.
