Personal Health Record

A Health Record that Patients Keep Returning to Use

The IHR's Personal Health Record, references the same Individual Health Record record used by providers – but renders the record in language and terms a lay individual can understand (e.g., otitis media = ear infection).  So when the individual records a blood pressure or a new allergy, it immediately becomes part of the IHR that the physician also uses.

Conversely, if a clinician renews a medication or orders a new test, that information is known to the individual because there is only one record used by all stakeholders to support the well-being of that individual. Identifiers on each piece of data clearly indicate the source (e.g., clinician or individual) of the information enabling the determination of the significance of every contribution to the record.
