Individual Health Record

Individual Health Record (IHR) is an account of what matters most to the care and well-being of an individual. By design, it is made available when and where needed to all those that should have it in a maximally useful and fully interactive form. The IHR enables individuals and their caregivers to make informed choices about their health using all available information at the time choices are made. It transforms, repurposes, and incorporates the information from diverse source systems (including provider, health insurer, individual and caregiver-entered data) into a unified, individual-centric, interactive health environment for each individual.

IHR is much more than just a collector and viewer of health information. It incorporates innovative data management, rules and alerts, and workflow enhancing capabilities including a comprehensive health ontology that allows a generational leap in the delivery and management of health and healthcare.

Individual Health Record (IHR):

  • Obtains information from any and all healthcare information systems and sources, such as EHRs, pharmacies, laboratories, imaging, medical claims and other health record systems;
  • Transforms, repurposes, and incorporates information from these diverse sources into a coherent account of each individual's overall health;
  • Incorporates a comprehensive health information model whose schema sorts through the fragmentation of multiple transactions related to the same event (e.g., prescription, refill, claim, etc.) and creates a single, clean and ideal health record that is understandable;
  • Engages individuals in understanding and participating in their health and care and enables caregivers and individuals to interact using the same record in different ways;
  • Makes it easier to understand each individual's health by presenting their health information along with personalized health knowledge in differing contexts to individuals and their caregivers in ways that are useful and usable; and,
  • Leverages industry standard guidelines (e.g., NICE) to generate real-time alerts and reminders to all stakeholders at the point of care.

Electronic Health Records (EHR), Health Information Exchanges (HIE) and Personal Health Records (PHR) are regarded in the healthcare marketplace as tool sets that may be helpful.  However, healthcare organizations are realizing that these various niche systems are not useful unless the data that is contained in all of them is brought together in an understandable and reusable format.  IHR takes all data from any sources and creates a single health record in a single database that is then used to generate disease management plans, population health programs and real-time communication that changes the way care will be delivered for years to come.
